Job Summary
To support a growing Population Health team, a New Jersey Licensed Nurse Practitioner will provide high-quality, comprehensive follow-up care via phone and video to patients post-hospital discharge, working part-time (24-36 hours/week) in a remote setting.
Key responsibilities
- Manage telephonic outreach and scheduled visits for patients recently discharged from the hospital
- Conduct medication reconciliation, symptom screening, and coordinate follow-up care with various healthcare settings
- Collaborate with nursing and administrative teams to achieve individual and team-level population health goals
Required qualifications
- Certified Nurse Practitioner in Adult, Family, or Geriatrics with active NJ state licensure
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
- Experience engaging Medicare and Medicaid populations with complex conditions
- Comfortable in a digital-first environment with proficiency in relevant technology
- Bilingual in English and Spanish is a strong bonus
