Job Summary
To support a growing Population Health team, a part-time Nurse Practitioner with an active New Jersey license will provide telephonic and video-based follow-up care for patients post-hospital discharge, focusing on medication reconciliation, symptom screening, and care coordination.
Key responsibilities
- Manage telephonic outreach and scheduled video/phone visits for recently discharged patients as part of the Transitions of Care program
- Conduct medication reconciliation, symptom screening, and order necessary follow-up labs, medications, or supplies
- Coordinate care across various healthcare settings and collaborate with the nursing team to achieve population health goals
Required qualifications
- Certified Nurse Practitioner in Adult, Family, or Geriatrics
- Active state licensure in New Jersey, with the ability to obtain additional state licenses
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
- Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
- Comfort in a digital-first environment with experience in virtual care delivery
