Required Qualifications, Training, and Education

Bachelor's of Science degree in pharmacy from an accredited college or university
Pharm D from an accredited college or university
Active Unrestricted Pharmacist License required if making UM decisions; NJ License preferred
Submission of a Letter of Intent and Curriculum Vitae
Three Letters of Recommendation
