Job Summary

To support patient care management, the full-time New Jersey Licensed RN will assess clinical needs, coordinate care plans, and advocate for members while working remotely.

Key responsibilities

• Assess patients' clinical needs against established guidelines to ensure appropriate care levels
• Coordinate and implement care plans while monitoring services and negotiating fees as necessary
• Document patient care activities comprehensively and interact with multidisciplinary teams to ensure continuity of care

Required qualifications

• Active Unrestricted NJ RN License or active Compact License required
• Minimum of two (2) years of clinical experience
• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Proficiency in Microsoft Office products and basic knowledge of case/care management principles preferred
• Familiarity with health care contracts and benefit eligibility requirements preferred
