Job Summary

A company is looking for a Telephonic RN Nurse Care Manager.

Key Responsibilities

• Provide clinical coordination and manage a caseload of high-risk patients
• Collaborate with patients and multidisciplinary teams to develop individualized care plans
• Implement and analyze comprehensive clinical care management plans to address variances

Required Qualifications, Training, and Education

• Active R.N. License
• 2 or more years of direct home health experience or care management
• Ability to perform extensive telephone assessments
• Knowledge of Medicare regulations and home care standards
• Self-motivated, organized, and able to meet deadlines
