New Jersey Licensed RN Care Manager

Job Summary

New Jersey Licensed RN Care Manager, responsible for providing care management duties in a full-time remote position, ensuring appropriate levels of care and planning transitions in the continuum of care.

Key Responsibilities

• Assess patient's clinical needs against established guidelines to ensure appropriate care levels
• Coordinate and implement care plans while monitoring services and advocating for members
• Document care activities accurately and participate in quality improvement initiatives

Required Qualifications

• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Minimum of two (2) years clinical experience required
• Active Unrestricted NJ RN License or active Compact License required
• Proficiency in personal computers and MS Office products preferred
• Knowledge of case/care management principles and health care contracts preferred
