New Jersey Licensed RN Case Manager

Job Summary

To support patient care and management, the full-time New Jersey Licensed RN Case Manager will perform clinical assessments, coordinate care plans, and mentor new staff while working remotely.

Key responsibilities:

• Assesses patients' clinical needs to ensure appropriate levels of care and length of stay
• Coordinates and implements care plans, advocating for members and their families throughout the healthcare journey
• Monitors and evaluates the effectiveness of medical services and procedures, ensuring compliance with clinical practice guidelines

Required qualifications:

• Active Unrestricted NJ RN License or active Compact License required
• Minimum of two (2) years clinical experience and two (2) years healthcare payer experience
• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Proficiency in MS Office products and knowledge of case/care/disease management principles
• CCM certification preferred for Clinical Advocate roles
