New Jersey Licensed RN Case Manager

Job Summary

Managing a caseload of patients, the 100% remote New Jersey Licensed RN Case Manager will focus on kidney health management, delivering education on renal replacement therapy, coordinating care, and promoting high-quality patient outcomes.

Key responsibilities

• Handle inbound and outbound calls to provide exceptional service to members
• Educate patients on kidney health and related conditions, optimizing their understanding of dialysis and transplant options
• Collaborate with an interdisciplinary care team to coordinate patient care and address individual needs

Required qualifications

• Active, unrestricted RN license in New Jersey
• Bachelor's degree required; 6+ years of RN experience with 3+ years in case management may substitute for degree
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
