Job Summary

A company is looking for a Social Worker Care Manager (Hybrid) to support patients' mental and physical health care.

Key Responsibilities

• Support mental and physical health care for assigned patients, including screening and assessment for mental health and substance abuse disorders
• Provide patient education and facilitate referrals for psychosocial treatments and additional services as needed
• Document patient progress and treatment recommendations, and track clinical outcomes using a registry

Required Qualifications

• Minimum of MSW
• Preferred licensure or provisionally licensed
• Behavioral health experience and knowledge
• Must possess a valid driver's license and have an insured automobile in good working order
• Proficiency in using Excel and Word for daily assessments and productivity tracking
