New Jersey Licensed Social Worker

Job Summary

To support patients and families in a remote capacity, the per diem New Jersey Licensed Social Worker will conduct psychosocial assessments, provide grief support, and collaborate with an interdisciplinary team to develop effective care plans.

Key responsibilities

• Conduct comprehensive psychosocial assessments via video technology and telephone calls
• Provide grief and bereavement support, including anticipatory grief counseling
• Participate in weekly interdisciplinary team meetings to discuss and refine patient care plans

Required qualifications

• New Jersey LCSW, LMFT, or LPCC, required
• 2+ years of health care social work experience
• Experience in hospice, palliative care, oncology, or home health
• Palliative care certification, highly preferred
• Strong understanding of cultural competency with Medicaid population
