Job Summary
To support patients and families in a remote capacity, the per diem New Jersey Licensed Social Worker will conduct psychosocial assessments, provide grief support, and collaborate with an interdisciplinary team to develop effective care plans.
Key responsibilities
- Conduct comprehensive psychosocial assessments via video technology and telephone calls
- Provide grief and bereavement support, including anticipatory grief counseling
- Participate in weekly interdisciplinary team meetings to discuss and refine patient care plans
Required qualifications
- New Jersey LCSW, LMFT, or LPCC, required
- 2+ years of health care social work experience
- Experience in hospice, palliative care, oncology, or home health
- Palliative care certification, highly preferred
- Strong understanding of cultural competency with Medicaid population
