New Jersey Licensed Triage Nurse

Job Summary

A company is looking for a Triage Nurse - Clinical Navigation.

Key Responsibilities

• Field incoming calls and perform detailed assessments by phone, providing guidance using decision-support tools
• Assist patients in securing appropriate appointments and document interactions in the necessary platforms
• Communicate outcomes to the patient's Primary Care Practice and provide clinical support during low-volume work cycles

Required Qualifications

• Experienced RN with at least five years of patient and clinical experience (critical care/emergency department experience preferred)
• Registered Nurse from an accredited school of professional nursing
• BSN preferred
• Active NJ RN licensure
