Job Summary

A company is looking for a Triage Nurse - Clinical Navigation.

Key Responsibilities

• Field incoming calls and perform detailed assessments by phone, providing guidance using a decision-support tool
• Assist patients in securing appointments and document interactions and outcomes in appropriate platforms
• Act as a support resource for integrated service lines and perform outbound calls to address clinical concerns

Required Qualifications

• Experienced RN with at least five years of patient and clinical experience, preferably in critical care or emergency department
• Registered Nurse from an accredited school of professional nursing
• BSN preferred
• Active NJ RN licensure
