Job Summary

A company is looking for a Complete Weight Management Navigator, LPN with NJ license.

Key Responsibilities

• Serve as a point of contact for patients and referring physicians, facilitating access to clinical and supportive care services
• Coordinate patient appointments and follow patients throughout the care continuum, collaborating with healthcare team members
• Develop and present patient education programs and maintain positive relationships with key customers in the healthcare setting

Required Qualifications, Training, and Education

• A minimum of 2-3 years of customer service or call center experience as an LPN
• Graduate of an accredited LPN program
• Must possess a valid NJ LPN license
• Demonstrates knowledge of nursing skills, hospital practices, and standards
• Ability to use general office equipment, including a multi-line telephone system
