Job Summary

A company is looking for a Telephonic Case Manager/ RN II.

Key Responsibilities:
  • Assess patients' clinical needs and ensure appropriate care levels
  • Coordinate and implement care plans while monitoring services and outcomes
  • Mentor and train new RNs and serve as a subject matter expert for projects
Required Qualifications:
  • High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
  • Minimum of two (2) years clinical experience required
  • Minimum of two (2) years' experience in health care payer required
  • Active Unrestricted NJ RN License or active Compact License required
  • CCM certification preferred for Clinical Advocate roles
