Job Summary
A company is looking for a Telephonic Case Manager/ RN II.
Key Responsibilities:
- Assess patients' clinical needs and ensure appropriate care levels
- Coordinate and implement care plans while monitoring services and outcomes
- Mentor and train new RNs and serve as a subject matter expert for projects
Required Qualifications:
- High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
- Minimum of two (2) years clinical experience required
- Minimum of two (2) years' experience in health care payer required
- Active Unrestricted NJ RN License or active Compact License required
- CCM certification preferred for Clinical Advocate roles
