Job Summary

A company is looking for a Bilingual LPN Care Coach.

Key Responsibilities:

• Utilize specialized care management software to call Medicare patients with chronic conditions monthly
• Build rapport with patients to coach them on health improvement through SMART goals and self-management education
• Implement and update the Plan of Care, including medications, appointments, and preventive care measures

Required Qualifications:

• Fluent in both English and Spanish
• Current, unrestricted LPN Compact License or New Mexico LPN license
• 3-5+ years' experience as a nurse
• Proficiency with EPIC electronic health record and web-based applications
• Self-directed and able to work independently while meeting performance metrics
