Job Summary
To support a growing pilot program, a New Mexico Licensed Bilingual Telehealth Physician will conduct initial telehealth assessments for enrolled members, synthesizing medical and social histories to establish clinical priorities and actionable care plans, all while working remotely on a contract basis.
Key Responsibilities
- Conduct comprehensive telehealth initial visits for enrolled members (15-20 min)
- Synthesize complex medical and social histories and identify clinical priorities and risks
- Create concise, actionable clinical plans and handoffs for Community Health Workers, LCSWs, and care navigators
Required Qualifications
- MD or DO with active, unrestricted New Mexico license
- Board-certified or board-eligible in Family Medicine, Internal Medicine, Geriatrics, or related primary care specialty preferred
- Substantial experience with Medicare/Medicaid populations or complex chronic disease management
- Skilled at telehealth assessment and building rapport virtually
- Strong clinical judgment and comfort with complexity and ambiguity
