Job Summary
Working remotely within New Mexico, the full-time New Mexico Licensed Care Coordinator will coordinate care for justice-involved individuals, conducting assessments, developing care plans, and advocating for member needs to ensure quality outcomes.
Key responsibilities
- Provides care coordination for members with behavioral health conditions requiring intensive interventions
- Conducts comprehensive health risk assessments and develops care plans in collaboration with interdisciplinary teams
- Monitors and evaluates the effectiveness of care plans, addressing gaps and adjusting strategies as necessary
Required qualifications
- 3-5 years of experience in Social Work, Nursing, or a healthcare-related field, or relevant experience in lieu of a degree
- Experience in utilization management, quality assurance, or community health
- Knowledge of referral coordination to community and private/public resources
- Ability to analyze trends and make decisions requiring significant investigation and original thinking
- Valid driver's license in the state of New Mexico
