Job Summary

A company is looking for a Care Coordinator IV to facilitate a team approach in delivering quality care and services.

Key Responsibilities

• Facilitates a team approach with the Interdisciplinary Care Plan team to ensure effective and quality care delivery
• Conducts in-depth health risk and comprehensive needs assessments, addressing various member needs
• Provides care coordination for members with chronic or complex conditions, implementing and monitoring strategies for improved health outcomes

Qualifications

• Masters degree with 4 years of experience, Bachelors degree with 8 years, or Associates degree with 9 years of experience; 12 years of experience may substitute for education requirements
• Proficiency in Microsoft Word, Excel, and Outlook is required
• Experience in analyzing trends based on decision support systems
• Business management skills, including cost/benefit analysis and negotiation
• Knowledge of referral coordination to community and private/public resources
