Job Summary

Working remotely within New Mexico, the full-time New Mexico Licensed Care Coordinator will manage care coordination for clients with behavioral health conditions, conducting assessments and developing care plans to ensure effective service delivery and improve health outcomes.

Key responsibilities
  • Coordinates care for members requiring intensive interventions, utilizing various clinical and community resources
  • Conducts comprehensive health risk assessments and develops individualized care plans, serving as the main point of contact for service delivery
  • Monitors and evaluates care plans, making adjustments as necessary to address gaps in care and improve member outcomes
Required qualifications
  • 3-5 years of experience in Social Work, Nursing, or a Healthcare-related field, or relevant experience in lieu of a degree
  • Experience in utilization management, quality assurance, or community health
  • Knowledge of referral coordination to community and private/public resources
  • Ability to analyze data and make informed decisions regarding care coordination
  • Valid driver's license in the state of New Mexico
