Job Summary

A company is looking for an LPN Care Navigator to manage patient care and provide health coaching remotely.

Key Responsibilities

• Manage an assigned panel of 400-450 patients with monthly outreach for continuity of care
• Conduct daily outbound calls to provide care plan support and health coaching
• Collaborate with clinical teams to address patient needs and close gaps in care

Required Qualifications

• Active Licensed Practical Nurse or Licensed Vocational Nurse license with a multi-state compact license
• Minimum 2 years of clinical experience
• Experience working with EMR systems
• Familiarity with chronic condition management and individualized care planning
• Intermediate to advanced computer skills and ability to navigate multiple systems
