New York Care Specialist

Job Summary

To support the Care Management team, the remote New York Care Specialist will execute non-clinical aspects of care management, including data entry, member outreach, and coordination of care plans while working collaboratively with clinical staff.

Key responsibilities

• Perform accurate intake and data entry for Care Management requests and referrals across various communication channels
• Conduct telephonic outreach to members for program enrollment and support their ongoing care needs
• Collaborate with the interdisciplinary team to manage caseloads and update care plans for low and moderate risk members

Required qualifications

• Bachelor's Degree
• Medical Assistant certification preferred
• 1 - 2+ years of client-facing or data entry experience in a healthcare environment
• Proficiency in MS Office applications
• Knowledge of medical terminology and medical payment processes
