New York Care Specialist

Job Summary

To support care management processes, the remote New York Care Specialist will execute non-clinical tasks such as data entry, telephonic outreach to members, and collaboration with clinical staff while working a shift from 11:30 am to 8:00 pm.

Key responsibilities

• Perform accurate intake and data entry for Care Management requests and referrals
• Conduct telephonic outreach to members for program enrollment and support care interventions
• Collaborate with the interdisciplinary team to manage caseloads and update care plans for members

Required qualifications

• Bachelor's Degree
• Medical Assistant certification preferred
• 1 - 2+ years of client-facing or data entry experience in a healthcare environment
• Proficiency in MS Office and knowledge of medical terminology
• Ability to work a flexible schedule, including weekends, nights, and holidays as needed
