New York Care Team Associate

Job Summary

Supporting the care management team, the full-time Care Team Associate in Fishkill, NY will manage daily census and task coordination, assist with documentation processes, and provide excellent customer service while working primarily onsite with potential for remote work three days a week.

Key responsibilities:

• Document all member interactions in compliance with regulatory requirements
• Coordinate and monitor care management documentation queues for various programs
• Provide clerical support and assist in member scheduling and communication

Required qualifications:

• High School Diploma or GED
• 2+ years of administrative support experience
• 2+ years of medical terminology knowledge
• 1+ years of proficiency in Microsoft Office
• Must reside within a commutable distance to Fishkill, NY
