New York Certified Peer Specialist

Job Summary

A company is looking for a Peer Support Specialist to provide transformative care to patients through direct communication and support.

Key Responsibilities

• Outreach to patients for encouragement, goal progress check-ins, and support
• Review patient status and escalate urgent matters to care team providers
• Document patient interactions and coordinate care with the patient care team

Required Qualifications

• Current and valid New York State Certified Peer Specialist Certification (NYCPS)
• One year of Peer Specialist experience with adults aged 18+ with a mental health diagnosis
• Current or former recipient of mental health services
• Knowledge in recovery and overcoming challenges from a mental health diagnosis
• Ability to work independently in a virtual environment
