New York Licensed Advanced Practice Provider

Job Summary

To deliver fully remote text and video-based acute care, the full-time New York Licensed Advanced Practice Provider will manage comprehensive clinical care, including pediatrics and mental health, while collaborating with a dedicated support team.

Key responsibilities

• Provide video-based comprehensive clinical care encompassing acute care, pediatrics, and mental health
• Support physicians with Epic inbox management and educate patients on treatment plans
• Drive high-quality care focused on patient outcomes while collaborating with Care Team Members

Required qualifications

• 5+ years of clinical practice experience as a Family Nurse Practitioner or Physician Assistant in a Primary Care setting
• Licensed in New York
• Minimum 1 year of experience using Epic EMR
• Currently enrolled or eligible for enrollment as a Medicare provider
• Experience in primary care and behavioral health, with the ability to treat acute and chronic conditions
