New York Licensed Behavioral Health Manager

Job Summary

A company is looking for a Behavioral Health Care Manager to join their Collaborative Care team.

Key Responsibilities

• Coordinate patient care and serve as the primary contact for patients in the Collaborative Care program
• Conduct patient assessments, provide education on mental health disorders, and deliver brief behavioral interventions
• Document patient progress, track clinical outcomes, and adjust treatment plans as necessary

Required Qualifications

• Bachelor's degree in social work or a related behavioral health field
• Specialized training in brief psychosocial interventions and motivational interviewing
• Minimum of 2 years of experience in behavioral health or care management
• Preferred clinical license (e.g., Licensed Mental Health Counselor, Licensed Social Worker)
• Experience with screening for mental health and substance use disorders
