Job Summary

A company is looking for a Bilingual RN Case Manager.

Key Responsibilities

Assess member health status and coordinate care needs, including inpatient and outpatient management
Engage with patients, families, healthcare providers, and community resources to develop treatment plans
Monitor and adjust care plans based on individual health needs and treatment outcomes

Required Qualifications

5+ years of experience in clinical nursing and patient assessment
Bilingual in Spanish
Bachelor's degree or equivalent work experience
Graduate of an accredited school of nursing
RN - Registered Nurse - State Licensure with Compact State Licensure in good standing
