New York Licensed Bilingual RN Case Manager

Job Summary

To support healthcare services, the full-time New York Licensed Bilingual RN Case Manager will conduct comprehensive assessments, develop individualized care plans, and coordinate services remotely while ensuring effective communication with interdisciplinary teams.

Key responsibilities

• Conduct comprehensive telephonic and in-person assessments to evaluate medical needs and determine appropriate service referrals
• Develop and implement individualized care plans in collaboration with members, caregivers, and healthcare providers
• Coordinate referrals and advocate for members to ensure safe transitions of care and appropriate service delivery

Required qualifications

• Active, unrestricted RN license in the state of New York
• Associate's degree in Nursing; Bachelor's degree preferred
• Minimum of 3 years of case management experience in a hospital, home health, or ambulatory care setting
• Bilingual fluency in English and Spanish (writing, reading, speaking)
• Proficiency with electronic health record (EHR) documentation
