Job Summary
Supporting members in navigating their healthcare system, the full-time New York Licensed Care Coordinator will manage an assigned caseload, perform outreach calls, and document interactions accurately while working remotely.
Key responsibilities:
- Receive incoming calls from members regarding benefit questions, care coordination, and program inquiries
- Manage a caseload of members requiring short-term care coordination and health navigation
- Collaborate with the interdisciplinary care team to communicate member needs and monitor screening effectively
Required qualifications:
- Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training/experience
- High school diploma or GED
- Experience in a fast-paced environment managing multiple priorities
- Effective verbal and written communication skills
