New York Licensed Care Coordinator

Job Summary

To support comprehensive patient care, the full-time New York Licensed Care Coordinator will coordinate care, provide education and guidance to patients, and collaborate with care teams in a hybrid work environment that includes on-site visits and remote work.

Key responsibilities

• Coordinate care and develop comprehensive care plans in collaboration with the patient's care team
• Provide health education and advocate for necessary care and services to patients and their families
• Utilize behavioral change modalities to coach patients and caregivers on lifestyle changes that positively impact health

Required qualifications

• Bachelor's degree
• Experience in healthcare or community-based settings with diverse populations and complex chronic illnesses preferred
• Proficiency in MS Office and ability to learn hospital documentation and scheduling systems
• Commitment to engaging in community-based care service models
• Bilingual (English/Spanish) preferred
