Job Summary

A company is looking for a Care Manager, Bilingual Fujianese - 100% Remote.

Key Responsibilities

• Advocate, inform, and educate beneficiaries on services and health benefits
• Conduct assessments and develop care plans in alignment with treatment goals
• Coordinate care with multi-disciplinary teams and document all encounters accurately

Required Qualifications

• NYS RN or LCSW/LMSW (any state) for Medical Care Management
• 3 years of experience in a mental/behavioral health or addictions setting for Behavioral Health Care Management
• 1 year of pediatric clinical field experience for PEDS positions
• Fluency in Fujianese
• Experience managing member information in a shared network environment
