Job Summary

A company is looking for a Care Manager (Clinical) [LPN].

Key Responsibilities

• Manage and support patients' healthcare needs via telephonic outreach
• Monitor adherence to care plans and facilitate patient access to medical services
• Educate patients and caregivers about community resources and support self-management of disease

Required Qualifications

• Licensed Practical Nurse (LPN) with Unencumbered e-NCL Licensure
• New York licensure is required
• Minimum of 2 years of work experience in a healthcare setting with complex chronic disease states preferred
• Chronic Care Management experience preferred
• Proficiency with a personal computer and technical skills
