New York Licensed Care Manager

Job Summary

A company is looking for a Care Manager - Bilingual Spanish/English.

Key Responsibilities

• Advocates and educates members on services and self-management techniques
• Conducts assessments and develops care plans in collaboration with healthcare teams
• Coordinates care among service providers to ensure member needs are met

Required Qualifications

• NYS RN or LCSW/LMSW (any state) for Medical Care Management
• 3 years of experience in mental/behavioral health or addictions for Behavioral Health Care Management
• 1 year of pediatric clinical field experience for PEDS positions
• Must be fluent in Spanish
• Experience managing member information in a shared network environment
