Job Summary

A company is looking for a Care Manager, LTSS Care Management (Remote).

Key Responsibilities

• Provide care management and coordination for high-risk members, including assessments and identifying gaps in care
• Develop and implement care plans with specific goals and interventions in collaboration with members and the care team
• Ensure compliance with care management processes and maintain documentation in the Electronic Medical Records System (EMR)

Required Qualifications

• Bachelor's degree required
• Current active RN license is mandatory
• CCM certification preferred
• 4 - 6 years of clinical experience required
• Experience in case management, care coordination, or managed care is essential
