Job Summary

A company is looking for a Care Manager - Maternity.

Key Responsibilities

• Advocates for and educates beneficiaries on services and self-management techniques
• Conducts assessments to identify barriers and develop care plans in alignment with physician treatment plans
• Coordinates care with multi-disciplinary health care teams and documents all encounters appropriately

Required Qualifications

• NYS RN or LMSW, LCSW
• Relevant work experience preferably as a Care Manager
• Knowledge of community health practices for the frail adult population
• Experience managing member information in a shared network environment
• Proficient with Microsoft Excel and electronic documentation systems
