Job Summary

A company is looking for a Coordinator of Social Work - Remote.

Key Responsibilities

• Participates in care planning meetings and reviews documentation for quality
• Monitors field staff documentation and conducts coaching sessions as needed
• Coordinates care management services and collaborates with team members for optimal participant care

Required Qualifications

• Graduated from a Master Social Work program acceptable to New York State Education Department
• Minimum of two years of administrative experience in a relevant management capacity
• Experience working with a frail or elderly population preferred
• Current active and unrestricted license and registration in New York State required
• Bilingual preferred
