New York Licensed Care Manager

Job Summary

To support individuals in achieving their personal goals, the full-time New York Licensed Care Manager will provide comprehensive care management, conduct assessments, develop life plans, and coordinate health services in a hybrid remote setting.

Key responsibilities

• Complete comprehensive assessments and develop individualized life plans in collaboration with clients and their support networks
• Coordinate care and ensure timely access to medical and behavioral health services while monitoring individual needs
• Assist individuals in transitioning between levels of care and provide ongoing support during critical events

Required qualifications

• Bachelor's degree with two years of relevant experience, or a Registered Nurse license with two years of relevant experience, or a Master's degree with one year of relevant experience
• Experience in comprehensive care management and service coordination
• Knowledge of health information technology and electronic health records
• Familiarity with community resources and social support services
• Prior experience in a healthcare or social services setting is preferred
