New York Licensed Care Manager

Job Summary

Supporting patients through telephonic outreach and care coordination, the full-time New York Licensed Care Manager (LPN) will monitor care plans, facilitate access to medical services, and provide ongoing health education in a remote setting.

Key responsibilities:

• Manage and support patients' healthcare needs via telephonic outreach
• Document visits and monitor adherence to care plans using EHRs and technology platforms
• Educate patients and facilitate access to appropriate medical and specialty providers

Required qualifications:

• New York LPN licensure is required; Compact LPN is desired
• Minimum of 2 years of work experience in a healthcare setting with complex chronic disease states preferred
• Chronic Care Management experience preferred
• Proficiency with personal computers and experience with Microsoft Excel preferred
• Technical and analytical skill set with excellent communication and organizational skills
