Job Summary

A company is looking for a Care Manager, LPN (Active New York License Required).

Key Responsibilities

• Deliver telephonic care management for patients enrolled in Value-Based Care Programs
• Review electronic health records to identify gaps in care and develop patient-centered care plans
• Communicate health updates and treatment plans to patients, caregivers, and providers

Required Qualifications

• Active, unrestricted compact LPN license
• 2+ years of nursing experience in various care settings
• Knowledge of pathophysiology and treatment protocols for common health diagnoses
• Ability to analyze patient records and work in a remote environment
• Proficient computer skills and adaptability to technology platforms
