Job Summary

A company is looking for a Case Manager Registered Nurse (Remote, NY).

Key Responsibilities

• Engage with members telephonically to coordinate care, spending 50-75% of the day on this task
• Develop and implement individualized care plans based on comprehensive assessments and member needs
• Document care management activities accurately in the member's electronic health record and collaborate with the interdisciplinary care team

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in the state of NY
• Proficient in Microsoft Office Suite and capable of utilizing these tools effectively
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management, discharge planning, or home healthcare coordination experience
• Access to a private, dedicated workspace to conduct work effectively
