Job Summary

A company is looking for a Care Manager Registered Nurse (RN).

Key Responsibilities

• Develop personalized treatment plans and support a multidisciplinary team approach for prevention and health promotion
• Assess members' needs, document progress, and collaborate with clinical team members for further medical treatment
• Provide self-care education, facilitate Care Team meetings, and engage members in their healthcare journeys

Required Qualifications

• Currently licensed Registered Nurse in the state of practice
• Current certification in Basic Life Support for healthcare providers
• Bachelor's degree (BSN) preferred
• At least three years of experience as a Registered Nurse with specific conditions management
• Experience in Condition Management or relevant certifications are a plus
