Job Summary

A company is looking for a Care Manager Registered Nurse (RN).

Key Responsibilities

• Develop personalized treatment plans and support a multidisciplinary team approach for health promotion
• Assess members' needs and document progress in the electronic medical record
• Provide condition-specific self-care education and facilitate Care Team meetings

Required Qualifications

• Currently licensed Registered Nurse in the state of practice
• Current certification in Basic Life Support for healthcare providers
• Bachelor's degree (BSN) preferred
• At least three years of experience as a Registered Nurse with chronic condition management
• Experience in Condition Management or relevant certifications are a plus
