Job Summary

A company is looking for a Care Navigator with a New York Nursing License.

Key Responsibilities

• Conduct virtual consultations focusing on patient education related to chronic diseases
• Review and respond to remotely transmitted patient data and document communications in patient records
• Assist patients in setting up devices and collaborate with team members as needed

Required Qualifications

• Current and valid New York Nursing License (LPN/LVN)
• Preferred current Multistate/Compact Nursing License
• 3+ years of experience in cardiology, primary care, home care, or chronic care management
• Preferred experience in remote monitoring or virtual care management programs
• Must have a HIPAA-compliant and distraction-free remote work environment with reliable internet
