Job Summary

A company is looking for a Care Navigator with a New York License.

Key Responsibilities

• Conduct virtual consultations with patients focusing on education related to chronic diseases
• Review and respond to remotely transmitted patient data and document communications
• Assist patients in setting up their monitoring devices and collaborate with the care team

Required Qualifications

• Current, valid New York Nursing License (LPN/LVN)
• Preferred Multistate/Compact Nursing License
• 3+ years of experience in cardiology, primary care, home care, or chronic care management
• Experience in remote monitoring or virtual care management programs is preferred
• Must have a HIPAA-compliant and distraction-free remote work environment with reliable internet
