New York Licensed Care Navigator

Job Summary

Seeking a New York Licensed Care Navigator, the full-time position will manage a caseload of patients enrolled in chronic care management and remote patient monitoring programs, providing education and support to improve health outcomes in a remote setting.

Key responsibilities

• Conduct virtual consultations with patients, focusing on education related to chronic disease management
• Review and respond to remotely transmitted patient data, documenting communications in patient records
• Assist patients in setting up devices for remote monitoring and collaborate with the care team as needed

Required qualifications

• Current, valid, and in good standing New York Nursing License (LPN/LVN)
• 3+ years of experience in cardiology, primary care, home care, or chronic care management
• Experience in remote monitoring and/or virtual care management programs (preferred)
• Ability to work in a HIPAA-compliant and distraction-free remote environment
• Reliable internet connection with a minimum of 100mbps download and 10mbps upload speed
