New York Licensed Care Navigator

Job Summary

Dedicated to enhancing patient health, the full-time New York Licensed Care Navigator will support patients enrolled in chronic care management and remote patient monitoring programs, conducting virtual consultations and collaborating with care teams in a remote setting.

Key responsibilities

• Conduct virtual consultations focusing on patient education related to chronic diseases
• Monitor and respond to remotely transmitted patient data, documenting communications in patient records
• Assist patients in setting up their monitoring devices and maintain ongoing communication to empower them in managing their health

Required qualifications

• Current, valid, and in good standing New York Nursing License (LPN/LVN)
• 3+ years of experience in primary care, home care, or chronic care management
• Experience in remote monitoring and/or virtual care management programs (preferred)
• Ability to work in a HIPAA-compliant and distraction-free remote environment
• Reliable internet connection with minimum speed requirements
