Job Summary

A company is looking for a Case Management Specialist (Remote).

Key Responsibilities
  • Engage with members telephonically to coordinate care, dedicating 50-75% of the day to this task
  • Develop individualized care plans in collaboration with members and maintain accurate documentation in electronic health records
  • Collaborate with the Interdisciplinary Care Team to address barriers to care and connect members with resources


Required Qualifications
  • 2+ years of experience in a health-related field
  • 2+ years of customer service experience
  • Proficient in Microsoft Office Suite and able to utilize healthcare technology tools
  • Access to a private, dedicated workspace for conducting work
  • Associate's degree in a healthcare-related field or Practical Nurse degree/certificate with active New York state licensure
