Job Summary

A company is looking for a Case Manager Registered Nurse.

Key Responsibilities

• Engage with members telephonically to coordinate their care, dedicating 50-75% of the day to this task
• Develop individualized care plans by compiling clinical information and collaborating with members
• Provide evidence-based disease management education and connect members with appropriate resources

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in the state of NY
• Proficient in Microsoft Office Suite
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management or home healthcare coordination experience
• Access to a private, dedicated workspace to perform job duties effectively
