Job Summary

A company is looking for a Case Manager Registered Nurse.

Key Responsibilities

• Engage with members and coordinate their care primarily through telephonic communication
• Develop individualized care plans based on comprehensive assessments and clinical information
• Document care management activities accurately and collaborate with the interdisciplinary care team

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in New York
• Proficient in Microsoft Office Suite and able to utilize technology for care management
• Access to a private space for remote work
• Minimum of 3 years of nursing experience and 2 years of case management or related coordination experience
• Associate's degree in Nursing (ASN) required; Bachelor's degree (BSN) preferred
