New York Licensed Case Manager RN

Job Summary

A company is looking for a Case Manager Registered Nurse (Remote, New York License).

Key Responsibilities

• Engage with members telephonically to coordinate their care and develop individualized care plans
• Provide disease management education and connect members with appropriate resources and support services
• Document care management activities accurately and participate in interdisciplinary team meetings to discuss member progress

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in the state of NY
• Proficient in Microsoft Office Suite
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management, discharge planning, and/or home healthcare coordination experience
• Access to a private, dedicated workspace for remote work
